The Baseline Method™ gives caregivers a consistent structure for documenting changes from an individual's usual baseline across eight observation domains. It organizes those observations into a timeline, a neutral pattern summary, and a printable summary you can bring to an appointment.

Caregiver observations are meaningful data. They are also distinct from verified clinical findings, and this tool labels them that way everywhere they appear.

• It does not diagnose, screen for, or imply any condition.
• It does not provide treatment recommendations or medical advice.
• It does not perform triage or determine urgency on your behalf.
• It does not use AI to interpret your entries. Summaries are simple counts of what you entered.
• No claim of clinical validation, effectiveness, diagnostic accuracy, or regulatory approval is made.
• Caregiver-entered vitals are self-reported observations, not validated clinical measurements.

The framework supports children and adults. Throughout the tool we use "person" or "individual" rather than only "patient," and we describe change relative to that individual's own baseline rather than to any population norm.

Records you create are stored securely in your signed-in account and are available on any device you sign in from. Only you can read them: every record is tied to your account and protected by database access rules that block anyone else, including anonymous visitors.

Nothing you type is sent anywhere before you sign in. Sign-in uses a one-time link or code emailed to you, so there is no extra password to manage.

This is a caregiver observation log — it is not a medical record, not an electronic health record (EHR), and not a diagnostic tool. Please avoid unnecessary identifiers: initials work well, and details you do not need should be left out.

This version does not upload or store media files. If you capture a photo or video, keep it stored securely on your own device and add a labelled reference to the episode so you can find the right file quickly during an appointment. References contain only the label and where you said the file is kept.

Each episode receives a descriptive level — Near Baseline, Mild Deviation, Significant Deviation, or Severe Deviation — calculated only from the fields you filled in. It is not validated, not diagnostic, not a measure of medical urgency, and does not indicate any condition. It exists to make changes easier to describe and compare over time.

Research participation is not active in this version. Nothing is shared, uploaded, or transmitted. Each record includes a place to note interest in a future de-identified contribution option, stored locally only. If such a program is ever offered it would be strictly opt-in and would require confirming consent at that time.
